Abstract:
Describes an initiative that was set up to: empower individuals, families, and groups to make informed health care decisions based on screening results; sensitize students to the health concerns of a multicultural community; promote community-based practice aimed at addressing unmet, identified health concerns; and create a sustainable community partnership. Students gain knowledge of cultural, socioeconomic, and healthcare problems through interacting with their clients. Clients gain knowledge of diabetes, hypertension, cardiovascular risk factors, and health promotion from the students.
