This instrument may be used to track the progress and measure the impacts and outcomes of a service-learning program or project. Areas are provided for the academic need and community goal that prompt the program/project, as well as the inputs (resources needed), activities, outputs (evidence of academic or content goals addressed), intermediate outcomes (evidence of changes in skills), and end outcomes (evidence of changes in student behavior or attitudes).
